THREAT FACTORS FOR INFERTILITY

There are several risk factors which in turn affect female and male fertility.
 
Main dangers which in turn bring about infertility are as below:
For Females:
  • Age >35 years
  • PCOS/ Irregular cycles
  • Hypothyroidism
  • Over weight, Lack of exercise
  • History of Appendicectomy
  • Very low menstrual flow or very heavy flow
  • Severe menstrual pain which may be due to infection or endometriotic cysts
  • Irregular cycles or lack of ovulatio
  • Recurrent infections
  • Presence of Diabetes or Hypertension
  • Late work schedules, stressful lifestyle, night duties
  • Tubal blocks
  • Anomalies of uterus
  • Fibroids uterus
  • Endometriosis or Chocolate cyst

 

For Males: 
  • Age group >40 years
  • Using tobacco
  • Alcohol ingestion
  • Being obese or perhaps lack of excercise
  • Using Laptop on lap or mobile phones in pant pockets
  • Over-exercising or No exercise
  • Sexually transmitted infections
  • Childhood chicken pox, mumps
  • Exposure to a number of drugs anti hypertensives, antidepressants, for gastric ulcer,etc.
  • Mental strain
  • Trauma to private area / surgeries in the past
  • Occupational exposure to heat.
  • Travelling in bike in hot sun for long time

Do you know obesity can cause lack of ovulation and also increase the incidence of infertility?

What is obesity?
   We should calculate BMI with height and weight of a women. According to guidelines, the
BMI >    kg/m2 is considered overweight and BMI > kg/m2 is considered obese. 
How can obesity affect fertility?
  Obesity can affect infertility from first step till the final stages of pregnancy. It can cause lack
of ovulation by changing hormone profile due to peripheral conversion or aromatization by body
fat. This in turn can cause hormone imbalance at the level of pituitary. 
  Not only ovulation is affected but also the egg quality, sperm quality in male partner is affected.
Hence even in IVF or test tube treatments the quality of oocytes are affected.  In all levels there
is effect of obesity from implantation to growth, hence the pregnancy rates are lower and
miscarriage rates are higher. Incidence of HT ( hypertension) , DM ( Diabetes) ,
IUGR ( Intra Uterine Growth Restriction) are higher in continuing pregnancy. 
So when to plan to pregnancy, should we have to wait till ideal body weight?
  If newly married or very young with no complications can take 6 months to slowly reduce the
body weight by proper diet counselling and exercise programs. But when a patient is already
having infertility, married for longer durations , age is more than 35 years, then waiting for
6 months to 1 year is not feasible and not worth wasting the time. For these patients, going
ahead after shorter break like 3 months and reducing body weight of upto 10 % or minimum
5 % is ideal as per guidelines. Prefect blood sugar and Hypertension control is mandatory.

Is Twin Pregnancy Good ?

  We know in fertility treatments the chance of twin pregnancy  is more when compared to natural conception. In most families the twin conception is celebrated with happiness. It is in fact cheerful and happy to have twin babies delivered but it is not so for all conceptions.
     The draw backs of twin pregnancy are:
  1. Need to take physical rest / quit working during pregnancy , particularly after 24 weeks
  2. Risk of miscarriage after 5th month due to weak uterine mouth or cervix. To prevent this there may be need to do cervical cerclage or uterine mouth tightening
  3. Risk of preterm labour or preterm fluid leaking, which leads to delivery before the dates and so admission in NICU. Admission in NICU often creates emotional and financial burden
  4. Increased incidence of increased BP during pregnancy
  5. There are identical twins who might have weight discrepancy . One of the twin might be underweight than the other

 

Due to these reasons, we try to explain patients who have good success rates, good embryos, weak uterus or previous pregnancy loss to go with single embryo transfer to have safe pregnancy and healthy child.

After IVF failure, whats next step

Everyone deserves a one of their own:
        There are 3 main methods of treatment to get pregnant. They are Ovulation Induction & Follicular Monitoring, Intra Uterine Insemination (IUI ) and  IVF / ICSI.
      Sometimes even the best methods don’t give the solution for the problems and it is really traumatic to have negative result after IVF.  But don’t worry, there are solutions
The problems are mainly divided into two,
  • Embryo related            
  • Uterus or endometrium related
Embryo related reasons, 
  1. Pre Genetic Testing for Aneuploidies
The embryos formed after IVF / ICSI is biopsied on day 5 blastocyst stage and then tested. Embryos normal on testing are transferred into uterus. There is also grading of these normal embryos based on mitochondrial activity which also in addition tells us which embryo to be transferred first.
 
  1. Growing upto blastocyst stage:
Embryos are generally transferred either on day 3 / cleavage stage embryos or day 5 / blastocyst stage embryos. Blastocyst stage embryo transfer helps to select which embryo has the best potential to grow. So blas tocyst transfer is better than cleavage stage embryo. 
 
 
From the first it is Zygote, two cell stage, four cell stage, cleavage stage or day 3 embryo and last is the day5 or blastocyst embryo.
 
  1. Protocols for injections or stimulation can be improved
So that good oocyte is formed.
  1. Better sperm can be produced by giving antioxidants, life style changes, followed by best sperm selection techniques for ICSI treatments.
Uterus or endometrium related reasons
  1. Hysteroscopy to diagnose and treat endometrial anatomical problems
  2. ERA or Endometrial Receptivity Array to select best time of embryo transfer 
  3. Use of PRP or Growth factors from our own blood after processing , to in still into uterus to improve endometrial blood flow and thickness and so better sticking of embryo to uterus. Reduces miscarriage or abortion rates also. 
  
With all these latest technologies, we can definitely ensure that all problems are sorted out and we get our own babies.
